Disc Prolapse: When Does It Need Surgery?

A disc prolapse is an advanced slip disc, where the inner disc material herniates and compresses a nerve. Many still improve without surgery, but an operation is recommended when leg pain is severe and persistent, when weakness develops, or when there are red-flag signs of serious nerve compression.

What is a disc prolapse?

In a prolapse, the soft centre of the disc pushes firmly through the outer ring and presses on a nerve root. It often causes stronger leg pain (sciatica) than a mild bulge.

Types of surgery

Most prolapses are treated with microdiscectomy or endoscopic removal of the herniated fragment — minimally invasive procedures that relieve the nerve with minimal disturbance to the spine.

When to seek urgent care: Bladder or bowel disturbance with numbness around the genitals is an emergency (cauda equina syndrome) — seek surgery without delay.
